Transconjunctival blepharoplasty is a surgical technique where the incision is made inside the lower eyelid (through the conjunctiva). This means there is NO visible external scar. The surgeon accesses the fat pads from behind the eyelid to remove or reposition them. This technique is ideal for patients with prominent fat bags but good skin elasticity.
Modern technique favors fat repositioning over removal. Rather than discarding the fat (which can create hollows later), the fat is released from its compartments and redistributed to fill the tear trough area below. This creates a smooth contour from the lower eyelid to the cheek. Dr. Kwang assesses which approach is best during consultation.
Mild eye bags can be camouflaged with strategically placed filler below the bag to smooth the contour. RF (radiofrequency) treatments can mildly tighten the skin. However, for moderate-to-severe eye bags with actual fat herniation, surgery provides the most definitive and long-lasting result.
Day 1-3: Swelling and bruising expected (this is normal). Day 5-7: Swelling begins to decrease significantly. Day 7-10: Sutures removed (if external approach used). Week 2-3: Most bruising resolved, can return to social activities. Week 4: Presentable appearance. Month 3: Final result visible.
With transconjunctival technique (incision inside the eyelid), there is NO visible scar at all. With external (subciliary) technique, the scar is placed just below the lash line and becomes virtually invisible within 2-3 months. Dr. Kwang recommends the transconjunctival approach whenever possible.
